This policy applies to our care-coordination text messaging program. It supplements, and does not replace, other applicable privacy notices or care-service agreements.
Information associated with our SMS program may include your name, mobile phone number, consent and messaging-preference records, opt-out requests, and message content and delivery records related to care coordination.
We use SMS-program information to send appointment reminders, assessment confirmations, caregiver scheduling updates, and follow-up check-ins. We also use it to respond to messaging questions and honor consent and opt-out preferences.
Our SMS program is for non-promotional care coordination, not marketing.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent will not be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.
Mobile information may be processed by messaging platforms, telecommunications providers, and other service providers only as necessary to operate our care-coordination SMS program. This does not authorize them to use your mobile information or consent for their own marketing.
SMS is not a secure channel for detailed medical information. Please do not send diagnoses, medical records, Social Security numbers, payment-card information, or other sensitive information by text.

Participation in our SMS program is voluntary. Providing a mobile phone number alone does not constitute consent to receive text messages.
You may withdraw SMS consent by replying STOP or contacting our office. Opting out of text messages does not cancel your care services.
Updates to this policy will be posted on this page with a revised effective date.
